I could not add the amount of the rebate which was for 12 one pound bottles. Then to list all 12 bottles on the form it will take all 12 numbers but only 4 show up on the line for serial numbers. The date I ordered the powder was October 13. When I click submit the form it dose not work. So what am I suppose to do? John Haugrud
