Overcharge. BS. You cannot overcharge a 223 to that extent, with the correct powder. Im pretty sure, that if this is factory ammo sure that this is not the issue. Most likely a projectile jammed in the throat from the previous, ejected round.

Was this a reloaded case, how many times ? I believe it was getting a case separation, from max charges and too many reloads and base of case separated on this firing.

Bullet left in barrel from squid round (low powder, no powder) from previous round.
Nothing is said about inspecting the barrel in this article.
